Lost Skippers Ticket

Ok fellow Fw'ers,
Old fart has lost his plastic Skippers Ticket, has paper one. Who do you get in contact with to get another one issued. He's looked everywhere for it, used to keep it in the boat with our crayfish and fishing licences but its gone in a nice small folder.
Some advice would be appreciated who to ring.
If stolen from boat, what the heck do we have to do, sign a Stat Dec or show the original paper one when getting another plastic Skippers Ticket.

I would guess the Dept of Transport would be the ones to contact. They will give you advice as to what to do.

still have my paper copy in the boat, very faded though.
the backyard holds both my skippers ticket and last years boat fishing licence, still yet to claim a credit card!!
just renewed the boat licence, when that and the skippers ticket arrive must remember to put the wallet up high away from 3 year old hands.
