Head down, most fish swallow
Head down, most fish swallow food head first to avoid choking on spines

As Hutch said.
Also, if you are looking at pinkies, they always attack whole fish by crunching the head first, as they don't have shearing type teeth. Sharp-toothed pelagics usually go for a cut 'em in half and deal with the results approach. Billfish stun with bill, grab across body, then turn around to swallow.

Doesn't matter
Doesn't matter which way you put them on, if a fish is going to take it he will grab it same as any dead bait.
Tried both ways when the bite is on and no difference, same goes for those who prefer half mulies using the head or tail is exactly the same.

Yip, don't think it really
Yip, don't think it really matters, I've nailed most my Dhuies tail down. After all fish don't swim backwards ;)

Heads up
i always fish heads up on snelled hooks, having said that 95% of the snapper I have caught have been on the top hook.
My spots are so secret even the fish don't know about them !
